An application to amend gender status must be completed and returned to the Community Housing team who will investigate using a Gender Change Investigation form.
Are contract holders required to provide proof?
Contract holders will be expected to provide proof for the legal change in name and a new birth certificate should be obtained to prove change in legal gender.
Contract holders who fail to provide the appropriate evidence to support their application should be notified and provided with 14 days to submit the evidence. Failure to submit within the time frame will result in the application not being processed
What do I do if relevant application and proofs are received?
An adjustment form must be sent through to the Rent Account Team ending the current contract holder and adding the details of the new name as the new contract holder (similar to assignment process). The previous contract name should be ended. Documotive will also require updating with the change in name and any information in relation to the name change must be recorded in the secure Documotive folder.
Do I need to notify the courts if legal action is ongoing?
A notification of contract holders name change should be made to the court at the earliest opportunity and any future documentation should be addressed to the contract holder in their new name.
Considerations
- Due to the sensitivity surrounding gender change, it is important that information relating to this should be stored securely and not placed on general accessible parts of the YODA system.
- Written notification should be sent to the contract holder to confirm their records have been updated
- If the contract holder is involved with any other team (Community Involvement, Support etc.) contact should be made with the relevant section in order to notify them of their new name.
- No reference must be made to the contract holder in their previous name, however information relating to previous contract may be made.
